"PRICING VALIDITY:

MONTHLY PRICING UNDER THIS AGREEMENT IS FIXED FOR THE DURATION OF THE CONTRACT EXCEPT FOR AN ANNUAL INFLATION ADJUSTMENT, LIMITED TO 3% OR THE CONSUMER PRICE INDEX (CPI), WHICHEVER IS HIGHER. ANY ADJUSTMENTS SHALL BE MADE ON THE FIRST YEAR ANNIVERARY OF THE DATE OF EXECUTION OF AN ORDER AND ON EACH SUBSEQUENT ANNUAL ANNIVERSARY THEREOF.

ANY AND ALL PRICING CONTAINED HEREIN WILL AUTOMATICALLY EXPIRE AFTER 90 DAYS AND NOT APPLICABLE FOR FUTURE ORDERS.

PRICING AFFILIATED WITH PRE-ORDERS FOR NEW PRODUCT LAUNCHES OR SPECIALS WILL AUTOMATICALLY EXPIRE AND BECOME INVALID AFTER 90 DAYS."

NOTE: Enrollment in the Biotricity Solutions presented above constitute a 2-year commitment and will auto-renew for an additional year on each anniversary thereafter. You may elect to opt-out of renewal with a 30-day written notice. A signature on this PO constitutes acceptance of Biotricity's offer to sell the products as listed and acceptance of the terms and conditions "setforth in the Master Service Agreement and monitoring services, if applicable, will be in accordance to the standard Physician Notification Criteria."

Biotricity can only provide monitoring of incoming patient data when devices are online, connected, and hooked-up appropriately. Device must be confirmed that it is online and getting proper ECG signal at start of study to ensure proper monitoring, complete data upload, and timely end of study reports.  Biotricity is not responsible for ensuring device connectivity and appropriate patient hook-up. 

Billing payment terms: Net 30, 1.5% per month penalty if paid after 30 days

Billings that are unpaid 70 days after the date of issuance will be subject to service restriction and/or suspension

WIRE FRAUD ADVISORY: If your clinic or business has a deposit or payment transaction with Biotricity (or “the Company”) and you receive an email containing NEW or REVISED wiring instructions, DO NOT respond to the email. Instead, call our billing account team or relationship manager immediately, to verify instructions prior to sending funds. DO NOT use any contact information provided in the suspected phishing email. Once requested by you, the Company will send specific wiring instructions directly to you in secure email communication. If you are ever in doubt about an email or wiring instructions, please call your billing account team or relationship manager at the Company.
